    Each row is one CDT code on this schedule: the schedule's allowance and where it bills — Insurance (Bucket A), the Patient (Bucket B), or Excluded (neither). Every allowance on a case flows from here. Stale schedules are the #1 cause of wrong quotes.

    Office fee schedule

    The office's own treatment fees — what every estimate starts from, before any insurance. There is one office fee schedule per practice (unlike carrier schedules). The treatment buttons on the chairside read from this list. Update it in place each year; each version carries an effective date and older versions are kept.

    Updating your fees — once a year, or whenever prices change (7.7c):
    1. Change the fees in the table below by hand, or scan in a new fee sheet and let it fill them (importing a sheet is a planned route).
    2. Set the new effective date above.
    3. Press Save as new version. The set you were using is kept under its own effective date, and you can open or bring it back from Earlier versions.
